Pancrelipase Market
Market Analysis
How the market stands today, and what is shaping it
The global pancrelipase market was estimated at approximately USD 1.94 billion in 2025 and is projected to reach around USD 2.81 billion by 2034, representing a CAGR of approximately 4.20% during 2025–2034, based on Core Market Research estimates. Treatment rests on porcine pancreatic enzyme replacement therapy, supplied as five marketed brands in the United States, four of them delayed-release capsules and one an uncoated tablet given with a proton pump inhibitor, all dosed on lipase units. Growth is supported by exocrine insufficiency arising from chronic pancreatitis, pancreatic cancer, cystic fibrosis and diabetes, by lifelong dosing once function is lost, and by prescribing rates that remain well below guideline expectations. Expansion is constrained by dependence on pig pancreas glands, by a shortage affecting the United Kingdom since June 2023, and by treatment costs that patients cite as their main reason for stopping. Competition runs on territory rather than differentiation, with the leading brand held by separate companies inside and outside North America. Phase 1a-1b results published in 2026 for a non-porcine engineered lipase mark the first credible route away from porcine sourcing.
Market Dynamics
Key market dynamics and their impact on the industry landscape
Market Drivers
- Exocrine pancreatic insufficiency follows chronic pancreatitis in 40% to 50% of cases and pancreatic cancer in 70% to 80%, and because pancreatic acinar function does not recover, each diagnosis converts into lifelong enzyme dosing taken with every meal and snack.
- An engineered microbial lipase reported in Gastro Hep Advances in 2026 digested fat comparably to porcine therapy at 10-fold lower dosing by mass and without enteric coating, and completed phase 1a-1b testing in healthy subjects and in patients with exocrine pancreatic insufficiency.
- The FDA required every pancreatic enzyme product to hold an approved new drug application from 28 April 2010, ending decades of unapproved marketing. The six products cleared under that framework hold a regulatory position that no unapproved preparation can now occupy.
- Pancrelipase is a chronic outpatient prescription, so United States demand runs through Medicare Part D and commercial pharmacy benefits. A Part D modelling study found 30-day out-of-pocket costs reaching USD 1000, which makes coverage design the gate on whether prescriptions are filled.
- United States Creon net revenues reached USD 1,512 million in 2025, a 9.3% increase. The brand is marketed by one company inside the United States and by another outside it, so commercial expansion is recorded on two separate sets of books.
Market Challenges
- All six approved products are extracted from porcine pancreas glands, so output is tied to pig slaughter volumes and to a biological purification process that cannot be scaled quickly. A global shortage has affected the United Kingdom since June 2023, and no approved non-porcine substitute exists to absorb the gap.
- Prescribing sits far below guideline expectations. English data show 48% of people with unresectable pancreatic cancer receiving enzyme therapy in early 2023 against a recommended 100%, and a review of 28 studies covering 14,874 patients put prescribing after acute pancreatitis at 17.8%.
- Cost removes patients who have already been diagnosed and started. Among 751 Indian patients with chronic pancreatitis, 22.1% were not complying with therapy, and high treatment cost was the reason given by 34.1%, ahead of negligence at 19.4% and adverse effects at 10.9%.
- The approved set has not grown since Ultresa in March 2012, and Ultresa itself was discontinued in 2016. A market with no new entrant in more than a decade has little internal pressure to expand capacity or compete on price.
- Shortage does not reduce demand so much as scatter it across presentations. In England, Creon 25,000 fell below predicted volumes while Creon 10,000 ran about 912 thousand capsules a month above prediction and Nutrizym tripled in secondary care, which complicates manufacturing planning for every supplier.
Opportunities
- A non-porcine enzyme could address both supply and acceptability at once. The engineered microbial lipase that completed phase 1a-1b testing in 2026 may, if later trials confirm it, remove the dependence on pig pancreas that underlies the current shortage.
- The gap between diagnosed and treated patients is larger than the treated market itself. Raising prescribing after acute pancreatitis from the 17.8% measured across 14,874 patients toward the rates seen in severe disease could expand volume without any new product.
- Diabetes may represent an unaddressed pool. A study of 182 people with diabetes detected exocrine pancreatic insufficiency in 18.1%, and systematic screening in that population could identify candidates who are not currently reached through pancreatic or cystic fibrosis pathways.
- Surgical pathways offer a defined point of initiation. The 2025 Brazilian consensus recommends starting enzyme therapy on clinical suspicion after digestive surgery rather than waiting for confirmatory testing, an approach that could be adopted elsewhere to shorten time to treatment.
- The Cystic Fibrosis Foundation published guidance in 2026 on designing non-inferiority trials for non-porcine products, which could lower the development risk for any company pursuing an alternative source and may shorten the path for a second entrant.
Trends and Insights
Key market trends shaping the industry landscape
Supply Has Replaced Demand as the Limit
High impactShort-termA global shortage beginning in the United Kingdom in June 2023 has made availability, rather than diagnosis or reimbursement, the immediate constraint on volume.
Insight: Forecasts built on rising prevalence may overstate near-term volume, because the constraint has moved to the porcine input and cannot be relieved by identifying more patients.
Substitution Between Strengths, Not Brands
Medium impactShort-termEnglish prescribing data show demand moving to smaller capsule strengths and to alternative brands rather than disappearing when a preferred presentation is short.
Insight: Demand in this market appears close to inelastic, which protects aggregate volume during disruption but makes individual presentation forecasts unreliable.
Non-Porcine Sourcing Reaches the Clinic
High impactLong-termAn engineered microbial lipase completed phase 1a-1b testing and was published in 2026, and the Cystic Fibrosis Foundation issued trial design guidance for non-porcine products in the same year.
Insight: A credible alternative source would change what the market is made of rather than how much of it is sold, and it is the only development identified that could reset the supply base.
The Treatment Gap Is the Real Headroom
High impactMid-termMeasured prescribing rates run from 17.8% after acute pancreatitis to 48% in unresectable pancreatic cancer, against guideline expectations that are considerably higher.
Insight: Most potential growth in this market comes from treating patients already diagnosed with a qualifying condition, which depends on referral behaviour rather than on new products.
Ownership Has Drifted Away From Pharma
Medium impactMid-termZenpep and Viokace moved to a nutrition company in 2020, and Pancreaze passed to a diversified holding company the same year.
Insight: Brands in this market are increasingly held by owners whose core business is not pharmaceutical development, which may reduce the likelihood of investment in new formulations from incumbents.
Segment Analysis
Market segmentation by product type, application, distribution channel, ingredient type
Five of the six pancrelipase products approved in the United States are delayed-release capsules whose enteric coating protects the enzymes from gastric acid. Viokace is the exception, a non-enteric-coated tablet indicated only in adults and only alongside a proton pump inhibitor that raises gastric pH. Capsule contents may be sprinkled on acidic soft food for patients who cannot swallow them, which is how paediatric dosing is handled without a separate presentation. No approved pancrelipase oral solution or powder was identified for the United States, so two of the four supplied Product Type values have no approved product behind them there. A randomised trial of non-enteric-coated therapy in 107 patients at the Asian Institute of Gastroenterology in Hyderabad was reported in Gut in 2026. It found no improvement in chronic pancreatitis pain against placebo at either 12 or 24 weeks, which limits the case for expanding the uncoated format beyond its current indication.
Geography Analysis
Regional market analysis
North America
Creon net revenues in the United States rose 9.3% to USD 1,512 million in 2025, the only quantified regional revenue growth identified anywhere in this market. That figure sits awkwardly against the stored global estimate of USD 1.94 billion for the same year, since it would imply that one brand in one country accounts for roughly three quarters of world sales. Either the stored figure covers a narrower scope than all pancreatic enzyme therapy or the market is extraordinarily concentrated, and no reconciliation of the two was available. The regional ranking rests on documented commercial and regulatory concentration rather than on a measured regional market size: all six approved pancrelipase products were cleared by the FDA, the 28 April 2010 approval requirement applies only here, and five brands remain marketed. A Medicare Part D modelling study found 30-day out-of-pocket costs reaching USD 1000, so coverage design rather than availability governs how much of that demand converts into filled prescriptions.
Europe
Europe is where the supply shortage has been measured most precisely. National English data covering January 2019 to February 2026 show the shortage beginning in June 2023 and reshaping what was dispensed rather than simply reducing it. Creon 25,000 stayed the most prescribed presentation at 12,192 thousand capsules a month in primary care but remained below predicted levels, while Creon 10,000 ran about 912 thousand capsules a month above prediction. Substitution moved to smaller strengths and to other brands, with Nutrizym more than doubling in primary care and tripling in secondary care, and Pancrex doubling in primary care. Prescribing was already below target before the shortage: the OpenSAFELY-TPP audit found 48% of people with unresectable pancreatic cancer receiving enzyme therapy in early 2023 against a recommended 100%, with regional variation inside England reaching 50% to 60% in the West Midlands. Germany contributes manufacturing capacity through Nordmark Arzneimittel.
Asia Pacific
Australia holds the longest continuous public record of enzyme supply in this region. Pharmaceutical Benefits Scheme data show dispensing rising from 33 to 85 dispensings per 100,000 population each month between February 2014 and October 2025, a 160% increase. The introduction of 60-day dispensing was not associated with a significant change in volumes, despite sustained shortages during 2025. India is the region's main source of clinical evidence and its clearest access constraint. A cohort of 751 patients with chronic pancreatitis found 166, or 22.1%, not complying with enzyme therapy, with high treatment cost cited by 34.1%, negligence by 19.4% and adverse effects by 10.9%. A survey of Indian gastroenterologists reported cost as the single biggest limitation and called for India-specific recommendations to address it. Chinese work is concentrated on surgical indications, including a multicentre randomised protocol in 204 patients addressing insufficiency after gastric cancer surgery.
Latin America
Latin American activity identified here is clinical rather than commercial. Six Brazilian surgical societies published a consensus in 2025 on diagnosing and treating pancreatic exocrine insufficiency after digestive surgery, recommending that enzyme therapy be started on clinical suspicion and judging faecal elastase-1 to be of limited value in the postoperative setting. Starting on suspicion rather than on a confirmed test result matters commercially, because the diagnostic step is where most eligible patients are currently lost. No regional market size, share or growth figure was identified for the region, and no pancrelipase product is approved or manufactured locally among those examined. Supply reaches the region through the international Creon franchise, which is held by a different company from the one that sells the brand in the United States, so regional volumes are not visible in the disclosure that covers the largest single market.
Middle East & Africa
No market size, prescribing rate or access study specific to this region was identified, and that absence is itself the main finding. One structural factor distinguishes the region from the others. Every approved pancrelipase product is derived exclusively from porcine pancreatic extract, and a 2024 Heliyon review of halal pharmaceutical alternatives identifies pancreatin among the ingredients for which Muslim-majority markets seek non-porcine substitutes. A 2025 qualitative study of Muslim patients found religious reasoning applied flexibly to prohibited ingredients according to how serious the illness was, with necessity invoked for severe conditions. The effect on a therapy for a chronic malabsorptive condition is therefore unlikely to be uniform, and it has not been quantified for enzyme therapy. The engineered microbial lipase that reached clinical proof of concept in 2026 is the first candidate that could remove the porcine question altogether, which may matter more here than in any other region.
Competitive Landscape
Key players and market positioning analysis
Competition here is organised by territory and by owner type rather than by product differentiation, because all five marketed products deliver the same porcine extract and are dosed on lipase units. The most unusual feature is that the leading brand is split between two independent companies. AbbVie Inc. holds Creon in the United States, where it reported Creon net revenues of USD 1,512 million for 2025, up 9.3%, while Abbott Laboratories carries Creon outside the United States within the gastroenterology line of its Established Pharmaceutical Products segment. The two separated in 2013 and now compete with nobody for the brand in their own territories, so the largest asset in the market never faces itself. Nestlé S.A. holds Zenpep and Viokace through Nestlé Health Science, acquired on 11 May 2020 as a Federal Trade Commission remedy rather than by competitive purchase; Zenpep recorded net sales of USD 237 million in 2018 under its previous owner. Viokace is the one non-enteric-coated tablet and is indicated only in adults with a proton pump inhibitor, which gives Nestlé the two ends of the format range. Vivus LLC supplies Pancreaze in the United States and Canada, bought from Janssen Pharmaceuticals for USD 135 million in 2018 and manufactured under contract by Nordmark Arzneimittel GmbH & Co. KG. Digestive Care, Inc., founded in Bethlehem, Pennsylvania in 1990, is the only independent specialist among them and markets Pertzye with Chiesi as its partner, differentiating on bicarbonate-buffered microspheres. Codexis Inc. is not a participant in current supply but is the most consequential name in development, having produced the engineered microbial lipase with Nestlé Health Science that reached clinical proof of concept in 2026. The principal competitive factors are access to porcine raw material, manufacturing reliability during shortage, and territory rights, none of which a competitor can overcome through clinical differentiation of the existing extract.

Industry Developments
11 recorded developments
Phase 1a-1b results for an engineered microbial lipase from Codexis and Nestlé Health Science were published in Gastro Hep Advances in 2026, showing a favourable safety profile and improved fat absorption. It is the first non-porcine candidate identified to reach clinical proof of concept.
An OpenSAFELY-TPP audit covering 22,860 pancreatic cancer cases, published in Seminars in Oncology Nursing in June 2023, found English prescribing of enzyme therapy rose from 41% in 2015 to 48% in early 2023, against a recommended standard of 100%.
A global shortage of pancreatic enzyme replacement therapy began affecting the United Kingdom in June 2023. Supply, rather than diagnosis or reimbursement, became the immediate limit on how much product reached patients.
The FDA approved a reformulated Pancreaze on 5 February 2020 extending shelf life to 36 months across all dosages, developed with contract manufacturer Nordmark Arzneimittel GmbH & Co. KG. VIVUS said the change would cut returned product and lower supply chain costs over time.
Nestlé Health Science completed the purchase of Zenpep and Viokace on 11 May 2020. The sale was required under a consent decree with Federal Trade Commission staff as a condition of AbbVie acquiring Allergan, so a competition remedy moved two brands to a nutrition company.
VIVUS emerged from Chapter 11 in December 2020 as a wholly owned subsidiary of Icahn Enterprises, continuing to supply Pancreaze alongside Qsymia. Ownership of a marketed pancrelipase brand passed to a diversified holding company rather than to a pharmaceutical acquirer.
VIVUS acquired United States and Canadian rights to Pancreaze from Janssen Pharmaceuticals for USD 135 million in June 2018. A pancrelipase brand changed hands as a standalone asset rather than as part of a therapeutic portfolio.
Digestive Care obtained FDA approval on 14 July 2017 for a 24,000 USP lipase unit Pertzye capsule, with microspheres of 0.8 to 2.2 mm. The company markets Pertzye to the United States cystic fibrosis community with Chiesi as its partner.
Ultresa was discontinued, reducing the marketed set to five brands. The withdrawal showed that clearing the FDA framework did not by itself make a pancrelipase product commercially durable.
Ultresa was approved on 1 March 2012 as the sixth pancrelipase product cleared under the new framework. It was the last new pancrelipase approval identified, so the approved set has not been added to in more than a decade.
From 28 April 2010 the FDA required every pancreatic enzyme product sold in the United States to hold an approved new drug application, ending decades of marketing without one. Preparations whose sponsors did not obtain approval had to leave the market, which fixed the number of legitimate suppliers.
